A sore knee starts small. You take the stairs one at a time. You park closer. You stop saying yes to things.
Regenerative Joint Clinics treats knees without surgery, at one office on Upper Riverdale Road. Our care is nurse practitioner led and physician supervised. We do not perform surgery, and we will not pretend that every knee can be fixed without one. What we can do is work through the non-surgical options properly, in the right order, with images of your actual joint in front of us.
Start with the diagnosis, not the treatment
There are several reasons a knee hurts, and they do not all respond to the same thing.
Osteoarthritis is the common one. The cartilage thins until the joint surfaces carry more load than they were built for. It shows up as morning stiffness, a deep ache after you have been on your feet, and swelling that comes and goes.
Meniscus wear catches, locks, or gives you a sharp twinge when you twist. In an older knee this is often gradual wear rather than a sudden tear, and a good number settle down without a surgeon.
A knee that feels loose on uneven ground usually points at a ligament. Bracing and a careful return to loading are normally the core of the plan.
Front-of-knee pain that gets worse kneeling or squatting tends to be a tendon or a bursa. Those are two different problems with two different treatments, so we work out which you have.
A knee full of fluid loses its range of movement quickly, and the swelling hides what is underneath it.
This is why the first visit is an evaluation rather than a treatment. We take your history, examine the knee, take images of it, and record a baseline score so that in three months we are comparing against something real.
What we can offer, and what each one actually does
Gel injections. Hyaluronic acid puts back some of the thick, lubricating fluid a worn knee has lost. It is the mainstay of our knee program. It does not rebuild cartilage and it does not stop arthritis. It makes a worn joint move more comfortably, and for many people that is the difference between a walk and no walk.
Cortisone. A steroid injection for a knee that is hot and swollen. Many patients feel a difference within a few days, some do not, and how long it lasts varies. It can buy you a quieter joint so you can sleep, move, and start rehabilitation. It wears off, and there is a sensible yearly limit per knee, so we plan when to use it rather than reaching for it every time.
PRP. We take a little of your blood, concentrate the platelets from it, and inject that back into the knee. Some patients with earlier wear or a tendon problem consider it. It is elective, most plans do not cover it, and you will know the cost before anything is booked.
Sarapin. A plant-based injectable with no steroid in it, which gives your provider a route when steroid is not the right choice.
Draining the knee. Taking the fluid off relieves the pressure straight away and usually makes whatever comes next work better.
An unloader brace. It shifts your weight away from the worn side of the knee as you walk. Where the wear sits mainly on one side, a brace can do as much for day-to-day comfort as an injection.
Physical therapy, by referral. We do not have therapists here. We set the therapy up with a practice near you and stay involved in how it goes.
What to expect afterwards
Expect some soreness at the injection site for the first week. Most people keep walking as normal. Go easy on deep squats, heavy lifting and long flights of stairs for a few days. If you were fitted for a brace, this is the week to get used to it.
Many patients notice a difference between two and six weeks, usually as less pain at the end of the day. Some do not. At three months we reassess against your baseline. If the response is not there, we change the approach rather than repeat something that did not work.
In our knee osteoarthritis program, about 90 percent of patients experience a 50 percent or greater improvement. Results vary by patient and by how far the arthritis has progressed.

Questions about knee treatment
Do I need a referral to be seen for knee pain?
Most patients call us directly. Some plans, particularly HMO, POS and certain Medicare Advantage plans, want a referral on file before they will pay. We check that when we verify your benefits.
How many visits does a course of gel injections take?
It depends which hyaluronic acid product your provider selects. Some are a single injection. Others are a short series spaced about a week apart. You will know which one applies to you before the first appointment is booked.
I have been told I need a knee replacement. Is it too late for this?
Not necessarily. We do not perform surgery. An evaluation tells you where you actually are. Some people are helped a great deal by these treatments and some genuinely need the surgeon. We will tell you which rather than sell you a course of injections.
Does any of this rebuild the cartilage?
No. Nothing we offer regrows cartilage or reverses arthritis, and you should be wary of anyone who says otherwise. These treatments reduce pain, calm inflammation, improve how the joint moves, and take load off the worn part. That is what they are for.
